Diabetes Prevention Program
A Year Long Program Proven to Prevent or Delay Type 2 Diabetes 

STARTING JUNE 2026

Transform Your Lifestyle Series

This 10‑week workshop series is led by Dr. Tammy Ouellette and Board‑Certified Health and Wellness Coach Kaylea Philobos. The program combines cognitive behavioral techniques and health coaching to support mental health, promote longevity, and build daily habits that support weight management. Through a personalized participant workbook and weekly group sessions, participants will reflect, journal, and practice strategies to create sustainable lifestyle changes aligned with their values.

Starts June 4th, 2026 - Join us on Thursdays at 6pm PST. ​​ | Cost: $30/week, or a copay if we are billing your insurance.

This series occurs once a month and is a dedicated space to explore important topics that support your mental, physical, and emotional well-being. Based on questions and concerns we commonly hear from our community, this program is designed to provide education, guidance, and practical strategies to help you feel more supported in your health journey. Each session will offer insight, practical tools, and time for Q&A to help you walk away feeling empowered and informed.

Join us on the third Tuesday of each month at 6pm PST for a one hour session.  | ​Cost: $30/month, or a copay if we are billing your insurance.
